Autopay Terms and Conditions Thank you for choosing to enroll in Cozzini Bros., Inc,’s (“Cozzini”) automatic payment program (the “Program”). Please begin by entering your (the “Customer”) preferred payment method into the fields below (the “Customer Account”). All payment information for the Customer Account will be tokenized and encrypted by Cozzini. DO NOT PROCEED WITH THIS ENROLLMENT BEFORE YOU READ THE TERMS & CONDITIONS OF THE PROGRAM (“Terms & Conditions”) BELOW. These Terms & Conditions are made with respect to the Program only. To review other terms and conditions applicable to Customer’s relationship with Cozzini, please refer to [other contract/website?] or contact Cozzini customer service. Upon certification of these Terms & Conditions and enrollment in the Program, Customer authorizes Cozzini to charge the Customer Account as payment for all services and products provided by Cozzini to Customer (the “Services”) during each delivery cycle. Payments under the Program shall be charged to the Customer Account at or after the time of each delivery. Cozzini hereby agrees that Customer’s invoices will include notice of any changes in services provided or service rates, including any price increases, which changes shall be clearly displayed on all invoices and in compliance with applicable law. If any payment for Services is rejected by Customer’s bank, credit card issuer, or other financial institution (a “Dishonored Payment”), Customer shall remain liable to Cozzini for such amount, and Cozzini reserves the right to charge the amount of any Dishonored Payment to Customer. If a payment for any Services is dishonored because of insufficient funds, then Cozzini shall notify Customer to update its Customer Account to designate a valid payment method. The Customer Account shall be charged on the third (3rd) day of the following month (and each month thereafter) until such Dishonored Payment is paid in full. If a payment for any Services is dishonored because Customer no longer desires to participate in the Program, Customer must contact Cozzini customer service to dispute such payment and withdraw from the Program. Customer is responsible for any uncollected amounts, plus any additional fees and other charges applicable to late payments, as allowed by applicable law. By enrolling in the Program, Customer consents to receive electronic confirmation of Customer’s enrollment and subsequent communications regarding the Customer Account via email. To terminate participation in the Program, please contact Cozzini customer service. It highlights Customer’s informed consent and signifies the Customer’s commitment to adhering to these guidelines and provisions (and all other terms, conditions and rules that Cozzini may implement, from time to time) throughout its enrollment in the Program.
